Please read these Terms and Conditions carefully before using the website at www.theconciergeclinic.com (the “Site”) or scheduling services with The Concierge Clinic (“we,” “us,” or “our”). By accessing or using the Site, you agree to these Terms. If you do not agree, please discontinue use of the Site.
1. Acceptance of Terms
These Terms are a legally binding agreement between you and The Concierge Clinic. We may update these Terms at any time without prior notice. The current version will always be posted on this page with its effective date, and your continued use of the Site after an update means you accept the revised Terms.
2. Our Services
The Concierge Clinic is an independent, nurse-practitioner-led medical aesthetics and wellness practice. Our services include injectables, skin rejuvenation, Esthetician services, skin health treatments, body and hair services, and wellness care. All medical aesthetic services are performed by or under the supervision of a licensed healthcare provider.
The information on the Site is for general educational purposes and does not constitute medical advice, diagnosis, or treatment. Using the Site does not create a provider-patient relationship. That relationship begins only after an in-person consultation, completion of required intake forms, and mutual agreement on a treatment plan.
3. Not a Substitute for Medical Care
Content on the Site, including treatment descriptions, FAQs, images, and wellness information, is educational and does not replace advice from a qualified healthcare provider who has evaluated you in person. Always consult a licensed medical professional before making healthcare decisions. If you are experiencing a medical emergency, call 911.
4. Scheduling, Appointments, and Cancellations
By scheduling an appointment through the Site or any linked scheduling platform, you agree to the following:
A valid credit card may be required to hold your appointment.
Cancellations with less than 24 hours’ notice, and missed appointments, may be subject to a scheduling fee as described in our Scheduling Fee, Late Cancellation, No Show, Late Arrival, and Refund Policies.
Arriving more than 15 minutes late may result in a shortened treatment or rescheduling, at our discretion.
Prepaid services and scheduling fees are non-refundable unless The Concierge Clinic initiates the cancellation.
We may decline, reschedule, or discontinue services to any individual at our discretion, including for clinical contraindications or safety concerns.
5. Informed Consent and Treatment Authorization
Before receiving any medical aesthetic service, you will complete an intake and informed consent process that includes your health history, current medications, known allergies, and prior aesthetic treatments. You acknowledge that:
All medical aesthetic procedures carry inherent risks, which will be discussed with you during your consultation.
Results are not guaranteed and vary based on individual anatomy, health status, lifestyle, and response to treatment.
Scheduling an appointment online is not consent to treatment. Consent is completed in person before your service.
You are responsible for providing accurate and complete health information. The Concierge Clinic is not liable for adverse outcomes resulting from withheld or inaccurate information.
6. Payment Terms
Payment is due at the time of service unless other arrangements have been made in writing. We accept major credit cards, debit cards, and other payment methods available through our scheduling platform. Pricing is subject to change without notice and may vary based on treatment complexity, product selection, and clinical assessment.
Gift cards and prepaid packages are non-refundable and non-transferable unless required by applicable law. They have no cash value and cannot be redeemed for cash.
7. No Guarantee of Results
Medical aesthetic outcomes are individual. Before and after images shown on the Site reflect specific patients in specific circumstances and are not a guarantee or warranty of similar results. Results depend on factors outside our control, including age, skin type, medical history, lifestyle, sun exposure, and individual response to treatment.
8. Text Messaging Terms
The Concierge Clinic offers text messaging to patients who opt in. Messages may include appointment reminders and confirmations, pre- and post-care instructions, and replies to your inquiries. With your separate consent, messages may also include occasional practice updates, events, and offers. Consent to receive text messages is not a condition of receiving care or making a purchase.

Message and data rates may apply.
Reply STOP to any message at any time to stop all text messages. You will receive one message confirming your opt-out.

Carriers are not liable for delayed or undelivered messages.
Your mobile number and text messaging consent are handled according to our Privacy Policy and are never shared with third parties for marketing or promotional purposes.
9. Intellectual Property
All content on the Site, including text, photography, graphics, logos, service descriptions, and brand elements, is the exclusive property of The Concierge Clinic or its licensed content providers and is protected by applicable copyright, trademark, and intellectual property laws.
You may not reproduce, redistribute, modify, publish, transmit, display, or create derivative works from any content on the Site without our prior written consent. Unauthorized use may constitute infringement and may be subject to legal action.
10. Before and After Photographs
Before and after photographs displayed on the Site are the property of The Concierge Clinic and are used with patient authorization. These images are protected by copyright and may not be copied, downloaded, redistributed, or used without express written permission. No identifying information accompanies published images unless explicit consent has been provided.
11. Privacy Policy
Your use of the Site is also governed by our Privacy Policy, which is incorporated into these Terms by reference. It describes how we collect, use, and protect your personal information, including information submitted through contact forms, scheduling platforms, email, and text messaging. By using the Site, you consent to the practices described in our Privacy Policy.
We do not sell, rent, or share your protected health information with third parties except as needed to coordinate your care or as required by law. All PHI is handled in accordance with HIPAA and applicable state privacy laws.
12. HIPAA Notice
As a healthcare provider, The Concierge Clinic is subject to HIPAA. Any health information you provide through the Site or in connection with your care is handled in accordance with our Notice of Privacy Practices, available upon request or at your appointment. Website inquiry and contact forms are not secure channels for transmitting protected health information, so please do not include sensitive medical details in them.
13. Third-Party Links and Platforms
The Site may contain links to third-party websites, scheduling platforms, payment processors, or social media pages. These links are provided for convenience only. The Concierge Clinic does not endorse, control, or assume responsibility for the content, privacy practices, or accuracy of any third-party site. Your use of third-party platforms is governed by their own terms and conditions.
Online scheduling is provided through a third-party scheduling system. The Concierge Clinic is not responsible for technical issues, data breaches, or service interruptions attributable to third-party scheduling or payment platforms.
14. Disclaimer of Warranties
THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. THE CONCIERGE CLINIC DOES NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF VIRUSES OR OTHER HARMFUL COMPONENTS.
15. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THE CONCIERGE CLINIC, ITS OWNERS, EMPLOYEES, CONTRACTORS, AND AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF THE SITE OR THE SERVICES, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
Our total liability to you for any claims arising from your use of the Site shall not exceed the amount you paid for services in the three (3) months preceding the claim. Some jurisdictions do not allow the exclusion or limitation of liability for certain damages; in those jurisdictions, our liability is limited to the greatest extent permitted by law.
16. Indemnification
You agree to indemnify, defend, and hold harmless The Concierge Clinic, its owners, employees, and representatives from any claims, liabilities, damages, losses, and expenses, including reasonable attorneys’ fees, arising out of or related to: (a) your use of the Site; (b) your violation of these Terms; (c) your provision of false, inaccurate, or incomplete health information; or (d) your violation of any applicable law or regulation.
17. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of Ohio, without regard to its conflict of law provisions. Any dispute arising under or related to these Terms shall be resolved through binding arbitration under the rules of the American Arbitration Association, unless prohibited by applicable law. You waive any right to participate in a class action lawsuit or class-wide arbitration.
Nothing in this section prevents either party from seeking injunctive or other equitable relief in a court of competent jurisdiction to prevent irreparable harm.
18. Accessibility
We are committed to making the Site accessible to individuals with disabilities in accordance with the Americans with Disabilities Act (ADA) and applicable web accessibility standards (WCAG 2.1). If you have difficulty accessing any content on the Site, please contact us and we will make reasonable accommodations.
19. Website Platform
The Site is built and hosted on Squarespace. While we take reasonable steps to protect the security and integrity of the Site, Squarespace’s hosting, data security, and platform terms also apply to your use of the Site. 20. Entire Agreement
These Terms, together with our Privacy Policy and any applicable service-specific agreements or informed consent documents, constitute the entire agreement between you and The Concierge Clinic regarding your use of the Site and supersede all prior agreements, representations, or understandings.
21. Severability
If any provision of these Terms is found to be invalid, illegal, or unenforceable, the remaining provisions will continue in full force and effect. The invalid provision will be modified to the minimum extent necessary to make it enforceable.
